Asbestosis is a form of pulmonary fibrosis that is a direct result from exposure to asbestos. Inhalation of the asbestos fibers create inflammation and alveolitis. Macrophages that become activated release oxygen free radicals that develop cellular apoptosis which contributes to further fibrosis and malignancy. This condition ultimately leads to lung cancer, mesothelioma, or pleural plaques. The clinical manifestations of asbestosis include dyspnea upon exertion, nonproductive cough, inspiratory crackles, hypoxemia, and a decreased lung volume.
Asbestosis is diagnosed through pulmonary function tests and confirmed with a CT scan. There is not a cure for asbestosis therefore the treatment plan is simply supportive care. This can include the use of bronchodilators, expectorants, supplemental oxygen, antibiotics, and pain medication. There are also alternative treatment options such as acupuncture, meditation, massage therapy, nutrition, herbal medicine, and homeopathic medicine.
Educational needs for a patient newly diagnosed with asbestosis is that this is a progressive disorder that over a period of time will ultimately lead to death. Supportive care can slow the progression however there is not a cure. Patients should be encouraged to stop smoking or avoid triggers that cause shortness of breath. Family members should be included within the education of this particular disorder as conversations regarding end of life care should be scheduled to ensure the patients wishes are followed.
